A participant’s medical history, communication style, home environment, support network, behaviours of concern, or previous service experiences can all influence engagement and support required. That’s why we developed the CARE Tool.

What is the CARE Tool?

The CARE Tool is an objective framework designed to bring greater clarity to each participant referral before allocation occurs. It helps us move beyond surface-level information and build a structured understanding of the participant’s presentation, context and potential complexity.

The tool explores eight key areas, including:

  • Biological and medical factors.
  • Psychological factors.
  • Social context.
  • Environmental considerations.
  • Communication and engagement factors.
  • Required supports.
  • Risks to others and behaviours of concern.
  • Forensic considerations.

Across these domains, referrals are assessed and rated as low, medium or high. This is not about labelling participants. It is about ensuring we understand the full picture so that supports can be delivered safely and effectively.

Why this matters for allocation

Allocation decisions should be thoughtful, not rushed. The CARE Tool gives our allocators and clinical leaders clearer visibility of participant needs, allowing us to consider:

  • Clinician experience and skillsets.
  • Capacity to manage complexity.
  • Current caseload balance.
  • Required supports or safeguards.

Rather than matching based purely on availability, we’re able to make informed decisions that prioritise fit.

Where increased support requirements or risk is identified, proactive measures can be implemented early. This may include additional supervision, team-based approaches, co-visits, safety planning or structured support from leaders.

This proactive approach helps reduce disruption later.

Supporting safer, more sustainable service delivery

Service sustainability matters for participants and clinicians alike.

When all the variables impacting a participant are not well understood at the point of allocation, it can lead to avoidable challenges such as mismatched expectations, reduced participant experience, clinician burnout, or changes in therapist mid-plan. We know consistency is important, particularly for participants with complex presentations or behaviours of concern.

By increasing visibility upfront, the CARE Tool helps us create more stable clinician–participant matches from day one.

It also supports:

  • Tailored intervention planning.
  • Appropriate safety controls when delivering community-based services.
  • Clearer stakeholder coordination.
  • Early identification of additional supports required.
